What is saw palmetto?

Saw palmetto (Serenoa repens) is a palm extract used for over a century for prostate and urinary symptoms. It inhibits 5-alpha reductase — the enzyme that converts testosterone to DHT — and has anti-inflammatory properties. It is the most popular prostate supplement globally, generating hundreds of millions in annual sales.

The STEP trial (NEJM 2006) and CAMUS trial (JAMA 2011) found no statistically significant benefit over placebo for moderate-severe BPH. Earlier smaller trials suggested modest benefit for mild symptoms. Honest context matters before spending money on it.
