IPSS Score Tracking Log
The International Prostate Symptom Score (IPSS) is the same 0–35 questionnaire urologists use to grade BPH severity. Score yourself monthly and watch the trend, not just a single number.
How to use this sheet
- 1Take the free IPSS quiz at mancore.net/advisor (or use your printed copy from your urologist) once a month, same week each time.
- 2Write your total score (0–35) and which symptoms changed the most since last time.
- 3Mild = 0–7, Moderate = 8–19, Severe = 20–35. A change of 3+ points in either direction is considered clinically meaningful.
- 4Bring this log to every urology visit — a downward trend after starting a treatment is the clearest evidence it's working.
